Hepatocellular carcinoma (HCC) presents a significant problem that necessitates a greater understanding of its underlying molecular complexity in order to improve diagnostics and therapies. Recent studies have shed light on the critical role that mitochondrial dysfunction plays in the development and course of HCC. Once thought to be primarily involved in the synthesis of cellular energy, mitochondria are now known to be key participants in the regulation of a variety of cellular functions that go beyond bioenergetics. The purpose of this (...) review is to elucidate the diagnostic and therapeutic implications of mitochondrial dysfunction in HCC by synthesising ideas from conventional and contemporary scientific literature. Within diagnostics, the discovery of mitochondrial biomarkers such as mutations in mitochondrial DNA and abnormalities in respiratory chain complex activities provides a state-of-the-art viewpoint for the timely identification and accurate description of HCC. In addition, non-invasive imaging methods that focus on mitochondrial function offer prospective means of improving clinical diagnostic accuracy. Moving on to treatment, a growing area of study investigates how mitochondria-targeted therapies might be used to treat HCC. Novel substances and repurposed medications with the potential to induce selective apoptosis in cancer cells, reduce oxidative stress, and restore mitochondrial homeostasis are highly promising. In the fight against hepatocellular carcinoma, an understanding of the complex interactions between mitochondrial dysfunction and HCC opens new diagnostic opportunities and paves the way for individualised treatment plans. Background: Context: Digital health interventions have become essential instruments in contemporary medicine, providing viable means of improving patient outcomes and healthcare provision. The goal of this study is to examine the impact, difficulties, and potential future directions of digital health interventions across a range of healthcare situations by synthesizing results from 12 carefully chosen studies. Aim: This study's objective is to thoroughly examine and summarize the body of research on digital health interventions, with an emphasis on acceptance and utilization as (...) well as implementation and efficacy. Method: Twelve pertinent papers published between 2016 and 2024 were found after a thorough search of electronic databases. Numerous digital health interventions were covered by these research, such as remote monitoring systems, telemedicine, and mobile applications. In order to find common themes, sub-themes, and patterns among the chosen research, thematic analysis was used in the data synthesis process. Results: A number of important themes emerged from the thematic analysis, including interoperability problems, patient engagement, healthcare provider acceptability, regulatory concerns, and usability issues. The sub-themes that fell under these categories emphasized the variables that affect the uptake and efficacy of digital health interventions. Conclusion: The synthesis of data emphasizes how digital health initiatives can increase clinical results, improve patient care, and streamline the delivery of healthcare. But in order to fully reap the benefits, major obstacles like interoperability problems, regulatory difficulties, and usability impediments need to be solved. The recommendations put forth aim to promote innovation and evidence-based practice in digital health by stressing user-centered design, improving digital literacy among medical practitioners, and encouraging interdisciplinary collaboration. Background: The prevalence of diabetes is rising worldwide, especially in poorer nations. It is anticipated to emerge as the sixth main cause of mortality globally by 2030. The factors linked to self-awareness in diabetes patients are still debated.Aim: To identify factors associated with self-awareness among diabetic patients attending Al-Zaher primary health care center (PHCC), Makkah Almukaramah City, Saudi Arabia 2024Methods: A cross-sectional study was performed using a randomized sample of diabetes patients visiting Al-Zaher Primary Health Care Center in Makkah AlMukarramah (...) city. The used questionnaire included demographic information and data on the patient's beliefs and attitudes toward diabetes mellitus (DM). The statistical analysis was conducted using SPSS. A P-value below 0.05 was deemed significant.Results: The study included 523 patients with T2DM in different genders, age categories from 20 to 70 years and education levels. The majority of respondents were married (79.3%). 53.5% of respondents were fully informed of their health conditions, however 52% were not confident in managing their diabetes. 60% of patients stated that regular diabetic follow-up every one to two months would assist in maintaining their health. 50.7% of respondents said there is a significant likelihood of getting diabetic issues in the future. While 50.9% of respondents felt that diabetes may be remedied with medicine, 47% of them thought that those with diabetes would have a shorter lifespan compared to those without the condition. 48% of patients selected meals optimal for their health, whereas 51.6% engaged in exercise at least three times weekly to enhance blood glucose regulation. Nonetheless, 51.1% of them sometimes missed doses of diabetic medicine. The average total score of patients' self-awareness about diabetes mellitus was 6.8±3.9, with a median of 8 (range 1-13).Conclusion: The study found inadequate self- awareness among diabetic patients. Male gender, younger age and high education level are the significant factors associated with high self-awareness toward DM. The study highlights the need for educational programs to improve self-awareness and empower patients, especially females, older individuals, and those with lower education levels, in Saudi Arabia. After almost fifty years of operation, government through a number of announcements declared that FELDA (Federal Land Development) schemes need to be revitalized so that it could play its role more effectively as a vehicle that would accelerate the country’s economic growth. Having raised this point, the major aim of this study is to examine the major socio-economic issues and the current socio-economic status of FELDA settlers.Information was gathered through face-to-face interview with the Mata Air FELDA settlers and the Rimba (...) Mas FELDA settlers in Perlis. Descriptive statistics such as mean, standard deviation and percentages were employed to describe the socio-economic problems and issues studied. The findings of the study indicate the implication of the ageing phenomenon of the FELDA settlers in Perlis to some extent affect the settlers’ ability to work effectively on their plantation. It is also found that the second generation issue has become the most significant factor contributing to the productivity and income increment of the FELDA settlers in Perlis as compared to other selected socio-economic variables. The result also suggested that, the ageing phenomenon, second generation issue and entrepreneurship problem must be seriously taken into consideration in order to accelerate the FELDA’s growth specifically and the country’s growth generally. Background: Nurse-patient communication is a unique clinical skill in the healthcare professions that promotes good quality care and patient outcomes. This communication can be disrupted by many barriers that impact the therapeutic relationship and deliver of care. Purpose: The study aims to identify the barriers affecting nurse-patient communication at primary health centers in Almadina Munawara City, Saudi Arabia. Methods: A cross-sectional study was performed among 212 nurses and 214 patients utilizing a self-reporting questionnaire. A version of the same file was (...) then uploaded into NVivo, and data that were coded under all communication barriers including language discordance among patients and healthcare providers (e. g., subtheme: patient prefers a mother tongue speaking doctor), cultural-religious difference and environmental cues between the involved parties to determine their importance in this context as well (see Table 2). Results: The results showed that language, cultural and environmental differences had an extensive influence on how nurses communicated with patients. Key barriers included nurse shortages, limited communication time and low income. Our results suggest that these barriers differ in their effect with language barrier and habitat disturbance being the most important ones. Conclusions: This study concludes that nurse-patient communication is a complex multi-sectional process, constrained by numerous barriers. Strategic intervention aimed at addressing these barriers can strengthen nurse-patient interaction and accelerate patient care. Purpose: The purposes of this study were to assess the knowledge, attitude, and practice of radiologists and paramedics regarding accident and emergency preparedness in hospitals in the southern region of the Kingdom of Saudi Arabia and how to improve their role. Materials and methods: This was a descriptive, cross‑sectional online survey that was carried out among radiologists and paramedics in the Kingdom of Saudi Arabia. A self-structured, close-ended questionnaire that was administered that consisted of 19 questions was included. The questionnaire (...) validity and reliability were evaluated for vetting and remarks. The questions were circulated through Google Forms, and it was circulated among the study participants through online mode. The data were collected systematically, and SPSS Statistics version 26.0 was used for data analysis Results: There were 139 responses, (77 specialist's radiology and 62 paramedics) participated in the study through Google Forms. In the study group, 84 (61.4%) were know the main steps for emergency response in major emergencies (p-value 0.000). Among the 19 questions used to assess the KAP, 12 questions were significant with a p-value less than 0.05. More than 72.6% agree the Management must be adequately prepared when emergencies and accidents occur. The respondents’ attitude toward emergency preparedness was generally positiveو Only a minority, that is, 43 (30.9%) of the respondents think it is possible for cooperation between radiologists and paramedics to interpret emergency cases and develop treatment plans in the emergency department. Conclusion: The level of knowledge was satisfactory among the radiologists and paramedics with a neutral level of attitudes, practice and knowledge regarding disaster preparedness. Follow-up research is essential to maximize the preparedness of the accident and emergency department. (shrink)

As ever more advanced digital systems are created, it becomes increasingly likely that some of these systems will be digital minds, i.e. digital subjects of experience. With digital minds comes the risk of digital suffering. The problem of digital suffering is that of mitigating this risk. We argue that the problem of digital suffering is a high stakes moral problem and that formidable epistemic obstacles stand in the way of solving it. We then propose a strategy for solving it: Access (...) Monitor Prevent (AMP). AMP uses a ‘dancing qualia’ argument to link the functional states of certain digital systems to their experiences—this yields epistemic access to digital minds. With that access, we can prevent digital suffering by only creating advanced digital systems that we have such access to, monitoring their functional profiles, and preventing them from entering states with functional markers of suffering. After introducing and motivating AMP, we confront limitations it faces and identify some options for overcoming them. We argue that AMP fits especially well with—and so provides a moral reason to prioritize—one approach to creating such systems: whole brain emulation. Hoping to bring some objectivity to the debate, Ben-Moshe has argued that conscientious objection in medicine should be accommodated based on its concordance with the ‘impartial spectator’, a metaphor for conscience drawn from the writings of Adam Smith. This response finds fault with this account on two fronts: first, that its claim to objectivity is unsubstantiated; second, that it implicitly relies on moral absolutes, despite claiming that conscience is a social construct, thereby calling its coherence and claims into question. Though not myself a physicalist, I develop a new argument against antiphysicalist positions that are motivated by zombie arguments. I first identify four general features of phenomenal states that are candidates for non-physical types; these are used to generate different types of zombie. I distinguish two antiphysicalist positions: strict dualism, which posits exactly one general non-physical type, and pluralism, which posits more than one such type. It turns out that zombie arguments threaten strict dualism and some pluralist positions as much (...) as they threaten physicalism—indeed, more so, since such positions need zombies to motivate them as alternatives to physicalism—and that the only pluralist position that escapes zombie arguments has a radically inflated ontology. Gauge invariance of a manifestly covariant relativistic quantum theory with evolution according to an invariant time τ implies the existence of five gauge compensation fields, which we shall call pre-Maxwell fields. A Lagrangian which generates the equations of motion for the matter field (coinciding with the Schrödinger type quantum evolution equation) as well as equations, on a five-dimensional manifold, for the gauge fields, is written. It is shown that τ integration of the equations for the pre-Maxwell fields results in the (...) usual Maxwell equations with conserved current source. The analog of the O (3, 1) symmetry of the usual Maxwell theory is found to be O (3, 2) or O (4, 1), depending on the space-time Fourier spectrum of the field. We argue that the structure that is relevant to the description of radiation in interaction with matter evolving in a timelike sense is that of O (3, 2). The noncovariant form of the field equations is given; there are two fields of electric type and one (divergenceless) magnetic type field. This paper defends the view that discovering that our universe is fine-tuned should make us more confident that other universes exist. My defense exploits a distinction between ideal and non-ideal evidential support. I use that distinction in concert with a simple model to disarm the most influential objection—the this-universe objection—to the view that fine-tuning supports the existence of other universes. However, the simple model fails to capture some important features of our epistemic situation with respect to fine-tuning. To capture these (...) features, I introduce a more sophisticated model. I then use the more sophisticated model to show that, even once those complicating factors are taken into account, fine-tuning should boost our confidence in the existence of other universes. Partial/particular will (al-irādah al-juz’iyyah) and the creation of human acts are two issues related to the predestination belief. Nowadays, it is unarguably accepted that humans have volition. However, the controversy over the formation steps of human will and act does not seem to be settled. Māturīdīs’ approach, taken with the intent to allow some space for freedom for humans in their actions and based on the partial will and postulation that there is a part in human actions that are not (...) created nor subject to creation, could not be understood at times. When it comes to misunderstandings of this kind, Sadr al-Sharī‘a and Ibn al-Humām, who are among the Hanafī-Māturīdī kalam scholars, are mentioned. Both mutakallims are some of the eminent figures who left their marks on the Hanafī-Māturīdī school. They affected a significant number of scholars coming after them by the authentic ideas they presented. However, even though neither Sadr al-Sharī‘a nor Ibn al-Humām did regard humans as creators of their own actions, they are confronted with an accusation as if they vitiated the fact that God is the Absolute Creator. Therefore, it is of grave importance to reveal Sadr al-Sharī‘a’s ideas, which are represented in his “Four Premises” concerning the formation of an act and Ibn Humām’s thoughts put forward in his “Ījādu al-‘Azm al-Musammamah” and “Takhsīsu Irādah al-Juz’iyyah.”. Every year, unfavorable environmental factors significantly affect crop productivity and threaten food security. Plants are sessile; they cannot move to escape unfavorable environmental conditions, and therefore, they activate a variety of defense pathways. Among them are processes regulated by stress‐associated proteins (SAPs). SAPs have a specific zinc finger domain (A20) at the N‐terminus and either AN1 or C2H2 at the C‐terminus. SAP proteins are involved in many biological processes and in response to various abiotic or biotic constraints. Most SAPs play (...) a role in conferring transgenic stress resistance and are stress‐inducible. The emerging field of SAPs in abiotic or biotic stress response regulation has attracted the attention of researchers. Although SAPs interact with various proteins to perform their functions, the exact mechanisms of these interactions remain incompletely understood. This review aims to provide a comprehensive understanding of SAPs, covering their diversity, structure, expression, and subcellular localization. SAPs play a pivotal role in enabling crosstalk between abiotic and biotic stress signaling pathways, making them essential for developing stress‐tolerant crops without yield penalties. Collectively, understanding the complex regulation of SAPs in stress responses can contribute to enhancing tolerance against various environmental stresses through several techniques such as transgenesis, classical breeding, or gene editing. Esto se debe a una lectura sesgada de su teoría del derecho, que se explica, a su vez, por la imagen que el constitucionalismo ha construido en torno a Waldron, como un opositor de Dworkin. Este trabajo muestra que Waldron suscribe a una teoría “dworkiniana” (...) del razonamiento jurídico, la que opera como eslabón entre su teoría de la democracia y su teoría más reciente de la dignidad humana. Plant production is diverse in the Asir region, and there are many methods of analysis. Perhaps factor analysis is one of the most important statistical methods through which productive power can be measured in various geographical regions. In this study, factor analysis was used to measure the developments of plant production in the agricultural sector in the Asir region, Saudi Arabia. The factor analysis showed the presence of three main factors that together explained about %86.55 of the total variation in (...) the development of plant production in the governorates of the Asir region. A contemporary orientation of the hospital experience model must encompass the clients’ religious-spiritual dimension. The objective of this paper is to share a previous experience, highlighting at least five reasons hospitals should invest in this direction, and an equal number of steps required to achieve it. In the first part, the text discourses about five reasons to invest in religious-spiritual support programs: 1. Religious-spiritual wellbeing is related to better health; 2. Religious-spiritual appreciation is a standard for hospital accreditation; 3. To (...) undo religious-spiritual misunderstandings that can affect treatment; 4. Patients demand a religious-spiritual outlook from the institution; and 5. Costs may be reduced with religious-spiritual support. In the second part, the text suggests five steps to implement religious-spiritual support programs: 1. Deep institutional involvement; 2. Formal staff training; 3. Infrastructure and resources; 4. Adjustment of institutional politics; and 5. Agreement with religious-spiritual leaders. Arianne Shahvisi has argued that a doctor’s conscientious objection to abortion is a misuse of their authority which unduly burdens patients and, moreover, does not succeed in its aim of exculpating objectors from participating in perceived evil. We examine these claims in this response. First, we ask what the ‘conscience clause’ really requires of doctors and whether Shahvisi has interpreted it correctly. Second, we explore the notions of vulnerability and power in the doctor–patient relationship and cast doubt on Shahvisi’s claims (...) about these. Third, we tackle Shahvisi’s claim that conscientious objection is self-defeating because it entails remaing causally implicated in a chain of events leading to abortion. We show that this claim entails absurdity, then revisit the classical framework for the ethics ethics of co-operation to show that conscientious objection can suceed in exculpating objectors from perceived wrongdoing. Bu bağlamda mezhep-siyaset ilişkisi bilindik bir öncül olarak öne çıkmaktadır. Bu araştırma, Büyük Selçukluların yıkılış sürecinden Moğol İstilasına kadarki sürede Eş‛arîliğin tarihsel gelişimini irdelemektedir. Bilindiği üzere Büyük Selçukluların kuruluş döneminde Horasan’dan uzaklaştırılan Eş‛arîler, yükselme devriyle tekrar bölgeye çağırıldılar. Sünnîliği merkeze alan siyasetin neticesinde Eş‛arîlik ön plana çıktı. Dolayısıyla Büyük Selçuklular ve Nizâmiye medreselerini merkeze alarak Eş‛arî düşüncenin ilerleyişini açıklamaya çalışmak yaygın bir (...) kabul olarak ortaya çıktı. Selçuklu devletinin yıkılışından sonra Horasan ve Mâverâünnehir coğrafyasındaki çatışmalarla artan kaos ve istikrarsızlık dönemi yaşandı. The historical adventures of the sects, which are Islamic schools of thought, have a complex plane that includes different orientations rather than simple and one-way directions. Understanding and correctly interpreting this multi-layered factual dimension requires multiple perspectives, like the variable leg of the compass. As of their historical processes, sects determine their positions depending on political, economic, social, etc. This position draws a route according to its positive or negative relations with its interlocutors. Intellectual debates and works of rejection are the outward manifestations of the interlocutor. It is a common acceptance to try to explain the progress of Ash‛arī thought by centering on the Great Seljuks and Nizamiyyāh madrasahs. This research examines the historical development of Ash‛arism by focusing on the period after the collapse of the Great Seljuks. As it is known, in the Great Seljuk period, the relationship of Ash‛arism with the state did not have a linear progression, but with the support of the state, it had a position that paved the way for it. After the collapse of the Great Seljuks, political conflicts, wars, invasions and plunders increased in the region, causing a great chaos in the region. In this process, it is understood that the chaotic environment continues until the state authority is re-stored. (shrink)

Dualism holds that some mental events are fundamental and non-physical. I develop a prima facie plausible causal argument for dualism. The argument has several significant implications. First, it constitutes a new way of arguing for dualism. Second, it provides dualists with a parity response to causal arguments for physicalism. Third, it transforms the dialectical role of epiphenomenalism. Fourth, it refutes the view that causal considerations prima facie support physicalism but not dualism. After developing the causal argument for dualism and drawing (...) out these implications, I subject the argument to a battery of objections. Some prompt revisions to the argument. Others reveal limitations in scope. It falls out of the discussion that the causal argument for dualism is best used against physicalism as a keystone in a divide and conquer strategy. (shrink)

Some physical theories predict that almost all brains in the universe are Boltzmann brains, i.e. short-lived disembodied brains that are accidentally assembled as a result of thermodynamic or quantum fluctuations. Physicists and philosophers of physics widely regard this proliferation as unacceptable, and so take its prediction as a basis for rejecting these theories. But the putatively unacceptable consequences of this prediction follow only given certain philosophical assumptions. This paper develops a strategy for shielding physical theorizing from the threat of Boltzmann (...) brains. The strategy appeals to a form of phenomenal externalism about the physical basis of consciousness. Given that form of phenomenal externalism, the proliferation of Boltzmann brains turns out to be benign. While the strategy faces a psychophysical fine-tuning problem, it both alleviates cosmological fine-tuning concerns that attend physics-based solutions to Boltzmann brain problems and pays explanatory dividends in connection with time’s arrow. Prior to writing the Aeneid, Vergil had already found this world-conquering power in amor and labor improbus,1 which conquer all things. Love and unrelenting toil always step beyond their limits. Unlike Achilles, they rage on after the bodies are buried. Always unsatisfied, they cut deeper than the superficial, personal manipulations of sword and stratagem. These forces reach beyond the satisfaction of any finite objective, encompassing and reshaping the entire world. As Vergil is more than an epic poet, the Aeneid transcends the presuppositions of Homeric epic. Against the wider horizon of world history, the epic form becomes the contingent expression of an outmoded worldview, one now seen from outside itself and suspended in comparison with other possible alternatives. And so, though Vergil sings of arms and the man, it is not at all clear that he means to exult the armed man. The ambiguity of Vergil’s commitments has been expressed in Adam Parry’s reading of Vergil as counterposing “two voices” in the Aeneid, 2 a main voice of Roman triumph and a counter -voice questioning the Roman imperial project. This counter -voice ties Vergil to genres outside of epic, genres with their arion 28.2 fall 2020 44 limb-loosening and the care of history own often opposing views of the human condition. Sappho offers one such critique of epic from the erotic perspective of lyric when she asks if any military pageantry could really be more beautiful than the object of her desire (Sappho LP 16). Though the sentiment comes from Greek lyric, it perfectly applies to the crisis of value-commitments Aeneas faces across his journey, most obviously (but not exclusively) in abandoning Dido for Roman glory. The conflicting voices Parry found within the Aeneid are thus analogous to the contrast between epic and lyric, between a view of the world as violence redeemed by public renown and a view of the world as love contained within personal intimacy. Even in Vergil’s most quintessentially epic moments, the deaths of great heroes, an echo of the lyrical voice is present. A formula repeated at the beginning and end of the epic—solvuntur frigore membra—describes the introductory surrender of Aeneas during the storm (Aen 1.92) as well as the concluding death of Turnus at Aeneas’ own hands (Aen 12.951). The hero and his rival both fall with their “limbs slack in a chill,”3 one in desperate supplication, the other in death. What exactly does this mean? Do the limbs seize up in a kind of terror, stiffening in rigor mortis as frigore would suggest? Or is the stress on the solvuntur, as the limbs loosen with a free sensuality evocative of lyrical eroticism? On the most basic physical level, solvere describes the body as somehow coming apart and losing its unified form, yet in these uses it also suggests the opposite, a rigid stiffening of the body. There is clearly more than a physical account intended in each of these expressions. Some deeper semantic sense unites the physical descriptions. Vergil is not entirely innovating with this expression, as Homer uses a similar phrase to describe the deaths of Patroclus and Hector (Il. 16.856–57; 22.68). I aim to demonstrate how this epic formula draws upon a literary tradition which supplies the wider semantic sense necessary to contextualize these passages in Vergil. The epic poets are relying upon a George Saad 45 long-standing motif to describe the loosening of boundaries between individual and world in a moment of bittersweet release. The notion of lifestyle has recently attracted the attention of various scholars as a social science concept. For thousands of years, human beings attempted to realise and manage their lifestyles, and governments have tried to influence the lifestyles of their people. Nevertheless, the definition of lifestyle and its conceptualisation is relatively new. Lifestyle means the specific method of living of an individual, group or community. Lifestyles include a set of values, behaviours, moods and tastes that can refer to the interests, (...) opinions, behaviours and behavioural orientations of an individual, group or culture. The Islamic way of life follows a grand plan of behaviour according to which all life is a movement to get closer to God. This plan, however, is dynamic and always demands adjustment. In a desirable religious life, all actions and situations can be called worship. People's lifestyles and the path they choose for themselves are deeply related to their success in life. Therefore, the present study aimed to evaluate the effect of the Islamic lifestyle on the academic success of Russian Muslim students in 2021. In total, 2000 graduate students of Kazan, Russia were studied in this research. According to the results of mean analysis in SPSS, the academic success of the students who followed the Islamic lifestyle was reported to be higher than the mean level (more than 3 out of 5). Furthermore, according to the Structural Equation Modeling in LESREL, the Islamic lifestyle has a positive significant effect on the students' academic success (p-value: 0.83; t-value: 7.92). CONTRIBUTION: Amongst the various factors that can influence students' academic success, the current research focused on the Islamic lifestyle. The findings of the present study offered a new insight into the relationship between the Islamic lifestyle and academic success in Russia. (shrink)

The curricula of the undergraduate study programs on Zakat and Waqf management in Indonesia are still in their early stages of development. In this research, we aim to evaluate these curricula using a competency-based approach towards several study programs. Specifically, we compare the needs of Zakat and Waqf management with the curricula of undergraduate study programs in Indonesian universities. To fulfil our objective, we conduct several interviews with the program heads and practitioners. We also propose a particular curriculum for the (...) study programs on Zakat and Waqf management. For this purpose, we apply a comparative research method and engage in qualitative research. Our results show that some competencies are not accommodated in the current curricula to meet the expectations of the Zakat and Waqf management. Therefore, we argue that it is important to change these curricula content to achieve the expected competencies. (shrink)
